Why I Built Varicocele Underwear
By Daniel Leibnowitz, Founder
In 2016, I was diagnosed with a left-sided varicocele. Grade II. My urologist gave me the standard advice: stay active, avoid heat, and wear supportive underwear.
Simple enough — except when I went looking for underwear actually designed for varicocele, I found nothing. There were jockstraps. There were compression briefs built for athletes. There were generic “supportive” boxer briefs that either crushed circulation or did absolutely nothing for scrotal lift. Not a single product had been built with varicocele anatomy in mind.
I spent the better part of two years rotating through options that almost worked. Too rigid. Not breathable enough. Fine for the gym, useless for a ten-hour workday. The frustration wasn’t just physical — it was the feeling that men dealing with a condition affecting 15% of the male population were being completely ignored by the underwear industry.
So in 2019, I decided to fix that.
What We Built — and Why It’s Different
Varicocele Underwear wasn’t adapted from athletic wear or repurposed compression gear. It was designed from the ground up around three clinical priorities that matter for varicocele management:
Venous pressure reduction The ergonomic lift pouch provides gentle upward support that reduces gravitational pooling in the spermatic veins — the core mechanical problem with varicocele. No crushing. No constriction. Just targeted lift that holds its shape through a full day and repeated washing.
Scrotal temperature regulation Varicocele raises local venous temperature by 1.5–3°C, which directly impairs spermatogenesis. Our cooling mesh construction passively maintains optimal scrotal temperature throughout the day — whether you’re at a desk, in the gym, or behind the wheel for ten hours.
Post-operative safety Flat seam construction with zero pressure points makes this the only underwear I’d recommend from day one of varicocelectomy recovery. Urologists who recommend scrotal support post-surgery now have something worth recommending.
The fabric: 95% Modal, 5% Spandex. Ultra-soft, moisture-wicking, maintains elasticity after 50+ washes.
Ten Thousand Customers Later
Since launching in 2019, over 10,000 men across the United States, Europe, and worldwide have made Varicocele Underwear part of their daily routine. Many are pre-surgery, managing symptoms conservatively. Many are post-varicocelectomy, in recovery. Many are fertility-focused, trying to optimize every variable within their control.
What they tell us consistently: this is the first underwear that actually felt like it was made for their condition.
That’s the only metric that has ever mattered to us.
